2025 Nomination Form for Honorees

JUDGING CRITERIA

The selection will be made on the basis of all around contributions to the community and or Italian American Community. This can include but is not limited only for extraordinary performance in the individual’s business or employment. The nominee should have a consistent track record of accomplishments rather than being honored for a single act of merit; although a single contribution or achievement is not ruled out if it is of sufficient importance.

WEIGHTING

• The degree to which their services/actions have made a lasting or meaningful 

   contribution or impact on  the Italian American Community – 40%

• The length and degree of their service(s) – 30%

• The extent to which their service or action might be considered above and beyond 

   the call of duty – 30%

PRESENTATION 

The Award winners are honored at the annual Columbus Day Celebration Banquet in October.

HOW THE NOMINATION PROCESS WORKS

• The member of CDCC or general public making the nomination will fill out the form

   provided and submit it to the committee. (Forms available in October)

• The committee shall review the qualifications of the nominee to determine if all 

   qualifications for the award are met. The CDCC board will review and discuss 

   all nominees.

• CDCC Board will select the final honorees
